An employee was monitoring as a tree was felled. The tree hung up; an excavator pushed on it to clear it, and its top half snapped and sprung backwards toward the crew. Part of this upper section, about 4 inches in diameter, struck the employee in the back of the right leg as he was trying to get away. He suffered an open fracture of the tibia and fibula.

An employee was grinding meat when the machine jammed. The employee cleared the machine and retracted her hand when the sleeve of her smock got caught and triggered the clicker, causing the blade to come down. The employee suffered an amputation of their left ring, middle, and index fingers.

Employees were setting large sandbags along the edge of a retaining wall using an excavator to lift the bags into place. An employee was attaching the hook when his right index finger was caught between the hook and the nylon strap. When the load was lifted, the finger was amputated at the first knuckle.
